The earlobes that stick out can easily be set back closer to the head during an otoplasty. The scars are located on the back of the earlobe and are easily concealed. This usually completes the last step in an otoplasty or ear contouring procedure. If earlobe contouring is going to be performed as a stand alone procedure, the earlobes can be st back under local anesthesia.

Where are the incsions for an otoplasty?

The incisions for an otoplasty or ear contouring surgery are located in the sulcus or the back of the ear or in the junction of the back of the ear as it meets the skull. The scars are extremely well concealed- even with short hair.

How long are the scars for a breast lift?

The length of incisions for a breast lift completely depends on the amount of breast lift needed. The circumareolar lift only provides about a 2 cm to 2.5cm (about an inch) ofbreast lift. Beyond about an inch there is more puckering of the incisions and flattening of the breast mound and pulling on the areola leading to an ineffective lift.

If there is more than a 2 cm lift needed, the next option is the lollipop incision, and for significant lifting or excess skin of the breast the full anchor incision is used.

What are the types of incisions for breast lifts?

There are 3 main incision types for a breast lift:
circumareolar: around the border of the areola.
lollipop incision: around the areola and a vertical incision from the 6 o’clock position of the areola to the breast fold
full anchor: incision around the areola, vertical incision to fold, and incision in the breast fold

Can I get a Mammogram after a breast lift?

If you are age 40 or greater, have a family history of breast cancer, or have a breast lump, you should have apre-operative baseline mammogram prior to your breast lift surgery. Dr. Kole will order the mammogram for you. If you already get routine mammograms, it is recommended to wait 6 months after yourbreast lift surgery to get your next mammogram in order to allow time for healing and the swelling to come down, and then resume your normal mammogram screening schedule.

Will the neck fat cells treated by neck liposuction ever come back?

The neck fat cells treated by neck liposuction are removed from your body and will never return. These neck fat cells will not be replaced by other neck fat cells. With weight gain, the non liposuctioned fat cells of the neck will get larger as expected but will never have the same volume as the neck would without the liposuction.

Do I need to lose weight before a tummy tuck (abdominoplasty)?

Possibly. The best candidates for a tummy tuck are those in good physical condition with pockets of fat or loose skin that haven’t responded to diet and exercise. Tn addition, the muscles of the abdominal usually have a weakened bowed appearance. Slightly overweight people whose skin has lost some of its elasticity can be helped with a tummy tuck. Patients who intend to lose a lot of weight are also encouraged to wait before undergoing the tummy tuck procedure, since skin typically loosens after substantial weight loss and the fat layer becomes thinner after weight loss, which could result in a less than ideal tummy tuck result.

What is the recovery like after neck liposuction?

Dr. Kole applies a special elastic based neck wrap for the first post-operative night after neck liposuction. He sees his patients the next day for a wound check, and thereafter gives them a wrap to be worn for the first week. Exercise and strenuous activity should be avoided for the two weeks following the neck liposuction procedure. There is minimal discomfort from the neck liposuction procedure which is controlled by oral pain medication. Patients can usually transition to Tylenol after 1-2 days following the neck liposuction procedure.

Now, chin implants are being utilized to enhance the appearance of a person’s chin or to create symmetry and facial balance from a weak chin. Will I be able to go home the same day as my tummy tuck?

All Tummy Tuck patients are able to return home the same day of surgery. Significant advances have been made in outpatient anesthesia with less nausea and with the addition of temporary pain pumps, which slowly disperse local anesthetic directly into the incision for three to four days following surgery. This significantly reduces the amount of pain and discomfort during the immediate postoperative period as well as requiring less narcotics for pain management.

The most important thing to do in preparation for a Tummy Tuck is to prepare your family or caregiver and your home for your post-operative recovery. Make sure that you are going to be comfortable by preparing the area of the house where you will be staying. Performing a cosmetic rhinoplasty becomes more difficult with age. A rhinoplasty, except small touch ups or small dorsal hump reductions almost always performed with anesthesia. Many factors, including patient comfort, patient safety, and the potential for bleeding must be considered when selecting anesthesia for your rhinoplasty.

In my opinion, general anesthesia is superior to intravenous sedation in all respects for rhinoplasty surgery for one main reason-patient safety. With general anesthesia, a small breathing tube is inserted by the anesthesiologist and a balloon on the end of this tube is inflated. If the patient bleeds, the blood will travel down the back of the throat and can enter the stomach and lead to nausea and vomiting- or worse- wind up in the patients lungs and lead to a pneumonia. With intravenous sedation, the patients gag reflex is diminished by the sedative drugs. For these reasons, I prefer general anesthesia as compared to intravenous sedation for rhinoplasty surgery.

Where are the incisions made for blepharoplasty? Are there scars after eyelid surgery?

When performing blepharoplasty of the upper eyelid, Dr. Kole makes an imperceptible incision in the upper eyelid crease. These incisions are made in such a way that they will heal into a thin virtually hidden scars in the upper eyelid crease. During the eyelid surgery blepharoplasty of the lower eyelids, Dr. Kole makes a small incision on the inside of the eyelid, which results in no external scar whatsoever – the incision is ‘invisible’ or a small scar immediately under the lash line when there is extra skin and fat of the lower lid that needs to be removed.

Does insurance cover the cost of blepharoplasty?

If your blepharoplasty (eyelid surgery) is performed solely to achieve a cosmetic benefit, insurance will not provide coverage. In such instances where your vision is being impared by drooping of the upper eyelids, insurance may cover part of all of the cost of the procedure.In these cases a non invasive visual field study may need to be performed. Dr. Kole has insurance specialists on staff who can help guide you through this process.

What options do I have for incisions for a breast augmentation?

What is my recovery time after a breast augmentation?

The typical recovery time will be only a few days with the pain subsiding rather quickly. Strenuous activities, exercise and sex should be postponed for 2 to 3 weeks after surgery to minimize complications such as bleeding, excessive scarring and implant shifting.

How long do I need to stay out of work after my male eyelid surgery procedure?

Male patients are advised to take off at least five to seven days of work after their male blepharoplasty procedure. The uncomfortable part of the surgery will only last a day or two but the “social recovery” of bruising and swelling that is visible to others will last up to two weeks. You can hide the bruising with glasses or sunglasses and as long as you avoid drafts, the sun and wind, you may return to a non-physical job. To avoid explaining yourself to your co-workers, you may want to stay out for up to two weeks.

Can I still get mammograms with breast implants?

Yes, the mammogram technician can perform a mammogram with breast implants in place. Please tell the technician that you have implants so they can protect the implants from damage from the mammogram machine and also do an additional study called a “displacement” view so more breast tissue can be visualized by the mammogram.

How can I tell if my breast implants rupture?

With saline implants, a ruptured breast implant will result in deflation of the implant, absorption of the sterile saline by your body and an obvious volume change in your breast size. The silicone outer shell that held the saline will remain and have to be removed and replaced with a new implant. There is no repair or “patch” kit for breast implants.

Silicone breast implant rupture of the older style will result in a subtle or obvious change in shape and feel of the implant and the breast. These implants when ruptured can be diagnosed by mammogram, where a “linguini sign” of the crumbled shell can be seen on X-Ray. The newer cohesive implants or the newsest highly cohesive “gummy bear” style 410 implants when ruptured will not have any discernable difference in breast size or shape. These new implants rupture can not be determined by x-ray or mammogram but must be diagnosed by MRI only.

Can I breast feed with breast implants?

Breast implants will not affect the ability to breast feed because the implants are usually placed under the chest wall muscle. There have been some studies of the old type of silicone implants (prior to 1992) having silicone particles in the breast milk after they have ruptured, but these implants are not in use any longer. The areolar incision may sever some milk ducts but this is usually not a probem with breast feeding.

How long is my breast augmentation surgery going to take?

The surgical procedure for a straight forward breast augmentation takes about 1 hour. The use of silicone implants adds only a few extra minutes of time for their placement. Patients with asymmetrical breasts requiring two different sized implants or patients requiring skin manipulation (breast lifts) to address symmetry issues require longer operative times.

Will I need help at home after my breast surgery and for how long?

Yes, although not mandatory, it is advisable to have a friend or family member assist you for the first day and night after your breast surgery. They can help you with meals, medication management and other household activities. You should limit your activities after either a breast reduction, breast augmentation or breast lift to reduce your chance of post operative bleeding and complications.

How long will I be in pain after breast surgery?

Although everyone is different, the most discomfort is experienced within the first day or so. With oral pain medicine, strict avoidance of arm activities, the pain will greatly diminish in 2 to 3 days. These three stages have a clinical significance to the patient. During the inflammation phase, the scar is not very strong. Redness of the wound and scar lasts throughout the metabolic phase. The redness does not fade for at least six to eight weeks after surgery or injury. It is at this stage that the scar is increasing in strength.

Of course, not everyone heals from wounds at the same rate. Many factors affect wound healing, including diabetes, HIV, vitamin deficiencies, and smoking. Some patients have a persistently heavy scar called hypertrophic scar or keloid. An unusually large or unsightly scar may be modified by later scar revision surgery.

A wound is not truly healed for at least 1 to 1½ years. It is rare to revise a surgical scar at least until this much time has passed.

Can I have cosmetic surgery if I smoke?

Most people are aware that smoking can damage their health in dozens of ways. For surgery patients, one of the most significant problems is that smoking reduces the amount of available oxygen to the cells — and oxygen is vital for wound healing. Smoking constricts small blood vessels, which limits the flow of oxygen-rich blood to the cells.You should stop smoking for several weeks before your surgery. The use of nicotine gum and patches is no help, as the active chemical — nicotine — can interfere with healing the same way that smoking does.

Smoking and nicotine patches can be especially damaging in surgeries like tummy tucks or facelifts, in which the skin is partially separated from its underlying blood supply and pulled tight under tension. It is mandatory to stop smoking at least two months before these procedures are performed.

You may be tempted to tell your doctor, “Oh, I never smoke,” when you actually have not been able to quit. Lying to your doctor is never a good idea, and in this case it can be a disaster. The effects of smoking on surgical recovery are very serious and deceiving your surgeon will only make things worse.

Recent studies show that most people are happy with their improvement after a cosmetic surgery. However, certain groups of people are less likely to be satisfied with cosmetic plastic surgery. These include:

  • younger patients.

How long is the recovery after a facelift?

Recovery after a facelift has two components: the immediate post-operative recovery, as well as the “social” recovery.After your surgery, there will be some swelling and bruising — most of which will diminish over the first two weeks. Some discomfort will be expected and oral pain medicine is usually quite effective.

The social aspect of the recovery is that, although you may feel great after several days, you will still have some residual bruising and swelling. It takes extra time for this to decrease to a level so that you can be seen in public. Women may shorten this time with makeup.

How long does a facelift last?case varies.

One of the benefits of a facelift is softening the jowls of the jaw line and elevation of the cheeks. A facelift also reduces the creases between the side of the nose and corner of the mouth, removes and tightens loose and wrinkled skin, and elevates the cheeks back into a more youthful position.Typically a facelift can last 10 to 15 years. I had liposuction. Will my fat reappear somewhere else?

Liposuction completely removes fat cells from a specific area of your body.However, if you gain weight, the fat that remains in the other parts of your body will expand. So, it is best — if possible — not to gain weight after liposuction. Exercise and a good diet will help you maintain your healthy weight and appearance.

What is the best surgery to pass off to other people as having good genes?

The eyes are the first place to show your age.Eyelifts or blepharoplasty take away extra skin on the upper or lower lids. Also, the fat is removed and the muscle is tightened. The sutures are removed after approximately five days and recovery is quick. People often will not be able to detect you’ve had surgery.

I thought silicone implants were removed from the market because of leaks. Why are they still used?

A new type of silicone implants has been re-approved by the federal Food and Drug Administration since November 2007.New implants are no longer made of the type of liquid silicone that the old implants used. If you cut the new, improved implants in half or if they get punctured, the contents remain semi-solid and gelatinous. The material cannot explode or leak into your body.

After Botox injections, will my facial muscles lose the ability to work?

No.If you stop having periodic Botox injections, the muscle will eventually recover — and the frown lines will eventually return. You may find, however, that you need less Botox after repeated injections over several years.
