What is the difference between saline and silicone breast implants?

Both saline and silicone implants have the same basic structure: an outer inert silicone shell. The difference is that saline implants are filled with a sterile salt water solution that is the same as that used for intravenous fluid replenishment, Silicone breast implants are pre-filled at the factory with a cohesive silicone gel material.
